About South West District Palliative Care & Nav-CARE
Since 1997, South West District Palliative Care (SWDPC) has supported individuals and families in the municipalities of Rockwood, Rosser, Woodlands, St. Laurent, parts of Armstrong, and the towns of Stonewall and Teulon who are living with declining health or experiencing grief.
SWDPC offers compassionate, community-based support through volunteer programs, including Nav-CARE (Navigation: Connecting, Advocating, Resourcing, Engaging)—an innovative program that provides trained volunteer navigators to support people living at home with declining health.
Nav-CARE helps individuals stay connected, access resources, and feel supported earlier in their health journey. If you or someone you know may benefit from a volunteer visit, we encourage you to reach out.
We also welcome individuals interested in volunteering. Volunteer recruitment and training are ongoing, with flexible online training available.
SWDPC is a community-funded organization and relies on donations and local support to continue offering these services.
